Our Approach

We have two Continuing Care Units (traditionally called "Nursing Homes") of 76 beds each. Each unit provides a very high ratio of hands-on care givers to residents to ensure quality care.

Dignity begins with a special kind of staff - one with a sincere love and appreciation of older people. Our skilled nursing care is second-to-none in promoting wellness of the body, mind and spirit. After all, that’s been our hallmark since 1929.

We encourage residents to continue to enjoy the activities that give their lives meaning and joy. Family members are always encouraged to join holiday celebrations, activities and special events. Our programs include:

  • Individually tailored menus
  • A chaplain for spiritual care and guidance
  • Dailyactivities such as bingo, flower arranging and current events discussions
  • Special events such as visits from Seafair pirates
  • Trips to the community for shopping or entertainment
  • Holiday celebrations and birthday parties
  • More than 200 volunteers and auxiliary members provide hands-on service and fund-raising
  • Twenty-four hour a day nursing and nursing assistant coverage
  • Dietician on staff
  • In-house pharmacy, dental clinic and beauty salon
  • Individual social work attention

Our Long Term Care Admissions Coordinator

The Admissions Process


Step 1. Submit an application. You may wish to tour the facility as well.

Step 2. When a bed becomes available and you are the first person on the list you will be contacted.

Step 3. An Authorization for Release of Information form will need to be signed in order for the Admissions Coordinator to obtain medical information about you.

Step 4. The Admissions Coordinator will gather information about your medical needs from your physician for the Foss Home Continuing Care and/or Dementia Care Unit Director to review.

Step 5. If you are appropriate for nursing home placement, you will need to schedule an appointment with your primary care physician prior to entry into the Foss Home Continuing Care/Dementia Care Unit. Your doctor will need to write a new history and physical and admission orders for placement in the Foss Home Continuing Care/Dementia Care Unit. The Admissions Coordinator will fax the appropriate paperwork to your physician for completion. You may download an online application.

Step 6. The Admissions Coordinator will schedule the date of entry.

Documents required at time of admission to Foss Home:

  • Social Security Card
  • Medicare Card
  • Insurance Card (s)
  • Medicaid Coupon or check for $6,000.00
  • Durable Power of Attorney for Finances (if applicable)
  • Durable Power of Attorney for Health Care (if applicable)
  • Living Will/Directive to Physician (if applicable)
  • Signed admission agreement

Documents required if the resident is admitted from home:

  • A current medical history and physical (must be completed within the five day window prior to admit)
  • The Nursing Home Identification Screen

Other important reminders:

  1. If current physician will not follow the resident to Foss, then a physician must be found that will agree to follow the resident to Foss.
  2. Arrange for transportation (private car, cabulance or ambulance)
  3. Bring supportive equipment presently being used, i.e., cane, wheelchair or walker labeled with the resident’s name.
  4. Bring labeled personal items such as photos, paintings, TV, favorite chair (if space permits).
  5. Bring launderable clothing for five to ten days. If family plans to do personal laundry, notify staff and provide a lidded container for soiled clothing.
  6. All electrical items (clock, TV, razor) need to be checked by maintenance for safety.
  7. Living Will/Directive to Physician (if applicable)
  8. Signed admission agreement
